Boundary-Access Holonomy and the Quantum Reversible Quotient

Formulates standard quantum mechanics as the zero-record-holonomy quotient of asymmetric finite boundary access. Defines boundary-access holonomy, separates three holonomy sectors (coherent/record/leakage), proves a restricted quotient-consistency theorem, and supplies Mach-Zehnder stable-record and QEC directed-leakage operational models. Born-rule program is a companion target. Does not derive CP^{N-1}, the Born rule, or full Hilbert reconstruction.
